I, JOAN BURTON, Minister for Social Protection, in exercise of the powers conferred on me by sections 3 (adapted by the Civil Registration (Transfer of Departmental Administration and Ministerial Functions) Order 2007 ( S.I. No. 831 of 2007 )) and 61 (amended by section 75 of the Health Act 2004 (No. 42 of 2004)) of the Civil Registration Act 2004 (No. 3 of 2004), hereby make the following regulations:

Citation

1. These Regulations may be cited as the Civil Registration (Certified Extract of Register of Deaths) Regulations 2014.

Commencement

2. These Regulations come into operation on 11 August 2014.

Definition

3. In these Regulations “qualified informant” has the meaning given to it in section 37 of the Civil Registration Act 2004 .

Particulars to be included in certified extract of entry in register of deaths

4. The particulars to be included in the certified extract of an entry in the register of deaths referred to in sections 61 (1) and 61 (2) of the Civil Registration Act 2004 shall be any one or combination of the items specified in the Schedule to these Regulations.

Article 4

PARTICULARS TO BE INCLUDED IN CERTIFIED EXTRACT OF ENTRY IN REGISTER OF DEATHS

1. Forename(s) of deceased.

2. Surname of deceased.

3. Former residence of deceased.

4. Date of death of deceased.

5. Place of death of deceased.

6. Date of birth of deceased.

7. Place of birth of deceased.

8. Birth surname of deceased.

9. Sex of deceased.

10. Civil Status of deceased.

11. Profession or occupation of deceased.

12. Forename(s), birth surname of father of deceased.

13. Forename(s), birth surname of mother of deceased.

14. Date of registration of death.

15. Profession or occupation of spouse or civil partner of deceased, if deceased was married or a civil partner.

16. Occupation(s) of parent(s) or guardian(s) of deceased, if deceased was less than 18 years of age on date of death.

17. Where section 42 applies—

(a) forename(s), surname, place of business, daytime telephone number and qualification of registered medical practitioner who signed certificate of cause of death, and

(b) forename(s), surname, qualification, address and signature of qualified informant.

18. If an inquest in relation to the death or a post-mortem examination of the body of the deceased was held, forename(s), surname and place of business of coroner concerned.

19. Name and address of registrar who registered the death.

20. Date of issue of certified extract of entry in register of deaths.
